GOVERNMENT CONTRACTS SURVEY FORM
For the months of January - June 2008, please provide the information requested below regarding federal, state, or local government contracts/subcontracts awarded to your firm as a result of information/assistance provided by the Kentucky Procurement Assistance Program (KPAP). The information can be submitted online by completing the appropriate fields below and clicking the "SUBMIT" button. The term "contract" includes all types of orders with government agencies, including purchase card/credit card buys and purchases from GSA schedules. For Indefinite Delivery/Indefinite Quantity (ID/IQ) contracts, list the dollar value of orders placed against your contract(s) during the current reporting period listed above. The term "subcontract" includes supplies and/or services that you provide to another firm to assist that firm in meeting their contractual obligations on a federal, state, or local government contract.
Architect/Engineer firms, please report the fee amount awarded to your firm rather than the total cost of the project (i.e., if your fee is 6% on a $1,000,000 project, enter $60,000 for the dollar amount of the contract).

Government Subcontracts awarded to your firm as a result of information/assistance provided by KPAP. For each subcontract listed
below, please provide the following information: name of the company awarding the subcontract to you (i.e., name of the company to which
you are providing supplies/services), brief description of subcontract, and dollar amount of the subcontract for your firm. If known,
please provide the name and location (city/state) of the government agency that your customer is contracting with, and the contract
solicitation/award number (2000 characters max for each subcontract listed; see bottom of page if you have more than 5 subcontracts to
report).
